| The
Humane Society, Animal League for Life is working to
eliminate the need to euthanize (put to sleep) healthy
homeless animals in Madison
County. We can help curb the increase in number of
unwanted animals through our Discount Spay/Neuter
Program. The program provides qualifying residents with the
means to have their pets altered at a much reduced rate.
This discount is offered through the
cooperation of the Humane Society, A.L.L., Madison County
area veterinarians, the Madison County Fiscal Court, and the City of
Richmond for residents of Madison County
only. Authorization of this certificate is
dependent on the availability of funds. More
than 80% of the funds used for this program must be raised
by the Society.

If you
have adequate income to have your pet spayed or neutered,
please do not apply. This program is intended for
low-income individuals and/or individuals on governmental
support (SSI, Disability, etc).

| How
To Obtain and Use the Spay/Neuter Certificate |
- You
may print a copy of our Spay
and Neuter Discount Application by Clicking Here (or
you may download a copy to your computer by right
clicking that link and clicking "Save Target
As..."), or you may pick up the Application for the
Spay/Neuter Discount in person at any Madison County Veterinarian
office. The application is very simple to fill
out.
- Complete and sign the
application.
- Send the completed
application with a stamped, self-addressed envelope to
the Humane Society, Animal League for Life, of Madison
County. P.O. Box 2094, Richmond, KY 40476.
- You should receive an
authorized Discount Certificate back from the society
within 30 days of the date received (usually sooner),
pending available funds.
- The certificate works just
like any "$ off" coupon. Present
your authorized certificate to your veterinarian at the
time you leave your pet for surgery.
Please note:
The authorized certificate is
only good for the animal indicated on your application and must
be used prior to the expiration date. Authorizations
are good for 60 days, after which, they expire. If the
certificate expires, you will need to reapply.
If this certificate cannot be
authorized by the Society within 30 days from the date received, it will be
returned and can be resubmitted at a future date.
There is a limit of 2
spay/neuter discount certificates per family per year.
Before making an appointment
for your pet, it is recommended that you discuss the total
pending cost of your animal's treatment with your
veterinarian, in advance.

The funds used to promote this
program must be raised through memberships, grant
requests, and various
fundraisers. Please consider showing your support
for the Society and the animals of Madison County by becoming
a member or by making a
